All new patients at Troutman Family Medicine are required to have the following documents on file at the office. Please copy and complete the forms, along with the required signature, and bring to the office at your initial visit. Please read and keep a copy of the TFM Office Policy and PHC Notice of Privacy Practice Act for your records. Thank you.
